New application form for suppliers with 5 or more employees

New version of Appendix 2 (version 8) now available (as of 1st July 2010)

Over the past few months we have been reviewing and updating Appendix 2 to make the application process easier for all. We hope you like the look and layout!

The old version of appendix 2 version 7.5 will only be accepted until 1st November 2010. Please make sure that if you are applying after this date (1st November 2010) that you use the new form.
